PLAY 2024-2025 Registration Form
To secure your child’s space a NON-REFUNDABLE registration fee as well as 1st months tuition is due at enrollment. Tuition is ONLY refundable if notice of non-enrollment/withdraw by parent is given 30 days before agreed upon start date. Should a parent delay agreed upon start date and then withdraw, tuition will not refunded.
